Nomination Form for Appointment / Re-appointment of LA Governors

Please download and use the following form to nominate, or re-nominate, someone to be a governor. The PDF version may be printed and returned by post, while the MS Word version may be completed digitally and returned by email, or printed and returned by post. (If returning the nomination by email, we will require the relevant signatures at a future date).
