Bringing Together Health and Social Care Services
for Older People and Disabled People
About
Bexley Care Trust
Bexley Care Trust is a new NHS organisation, formed on the 1st
October 2003. The organisation brings together:
- all the staff, services and functions of the former Bexley Primary
Care Trust, and
- some of the services previously provided by Bexley Council Social
Services for older people and disabled people, aged 18 and over.

The role of Bexley Care Trust
The role of Bexley Care Trust is to:
- improve the health of local people,
- provide, commission and develop health services for all Bexley
residents, and
- provide integrated health and social care services for older
people and disabled people who are 18 or over.
The Care Trust is responsible for the following social care services for older
people and disabled people aged 18 and over:
- Occupational therapy and rehabilitation services
- Social work
- Care management
- Screening service (first point of contact with social services for adults)
- Hospital social work
- Social care staff working in the rapid response team, the community rehabilitation
team and the community mental health team for older people
- Specialist HIV services
