Adult Social Care Pathways Transformation Consultation
The London Borough of Bexley is proposing to make changes to the way care services are delivered to adult residents. We want to ensure people needing services have access to the care and support they need by having:
- a life, not a service
- their needs identified and met at the right time and place
- choice and control over what is offered to address their needs
We would like to know what you think about our proposals which are outlined in the short Pathways consultation document A New Vision for Social Care (PDF, 440 KB).
