Better Care Fund and Integration Plan
The Better Care Fund is a government initiative, bringing existing resources from the NHS and local authorities into a single pooled budget to support the transformation and integration of health and social care. It requires NHS Bexley Clinical Commissioning Group and the Council to agree an integrated spending plan for how they will use their Better Care Fund allocation.
In Bexley, investment in integrated care reflects a shared ambition to improve the health and wellbeing of local people but also the need to respond to increasing demand and reducing budgets.
Our BCF Plan focuses on delivering better integrated and person-centred care with resources directed towards services based in the community, keeping people out of hospital unless they really need to be there.
Our BCF and Integration Plan for 2017 to 2019 was approved by the Health and Wellbeing Board in October 2017.
Bexley has worked as one of 16 local areas with NHS England and other national sector organisations to develop the Integrating Better guide. It captures the common features of good practice in integration between health, social care and the voluntary and community sector.
