New Adult Social Care Pathways

Bexley’s Adult Social Care pathway focuses on a strengths and asset-based approach, empowering communities to develop supportive strategies to support their members, creating links within the community and voluntary sector to develop sustainable supports for people in their own homes and places where they live.

The use of technology will further support people to be supported at home for longer with personalised targeted care and support being developed when required. Our aim is to strengthen people’s local bonds to develop a more community-based model with residential and nursing care being utilised only when necessary.

The Council intends to work closely with our providers to be innovative and creative in their approach to delivering adult social care now and in the future. Our implementation plan has five key principles – each of which requires partnership and collaboration with our providers to enable us to deliver for Bexley people.
