Use this service to:

  • Report an issue with a care service as a provider

Completion takes around 10 minutes.

Warning Don't use this form to report a safeguarding concern. Call 999 right away if someone is in immediate danger. 

Non-emergency concern

If you're worried about someone but it's not an emergency, call us at 020 8303 7777 (Monday to Friday, 9am to 5pm) and ask for 'screeners', or email screeners@bexley.gov.uk.

Before you start

You need to provide the following information:

  • the type of issue you are reporting
  • details about the issue
  • whether the issue has happned before
  • details of the resident the report relates to
  • your name, contact information, job title and provider name
