Introduction to preparing for adulthood

We support young people with an Education, Health and Care Plan (EHCP) between the ages of 14 to 25 years.

13 to 14 years

Your year 9 EHCP review will include outcomes to prepare you for adulthood

15 to 16 years

A worker from the Preparing for Adulthood (PFA) Team will attend your EHCP review We will ask if you would like a needs assessment to determine if you need support as an adult (18 years old).

16 to 17 years

The PFA worker will start your assessment. The assessment will tell us if you are eligible for support.

17 to 18 years

If you are eligible for support, the PFA worker will create a support plan with you. The support plan explains what you would like to achieve and who or what will help you to achieve it.

18 years

Bexley Adult Social Care will start providing the support agreed in your support plan.
