Bexley confirms new partnership to lead neighbourhood health services

Bexley has confirmed the appointment of its new neighbourhood health service integrator, following approval by the South East London Integrated Care Board.

The integrator for Bexley will be a partnership between the London Borough of Bexley, Bexley Health Neighbourhood CiC, Primary Care Networks, and Oxleas NHS Foundation Trust, who will act as the host NHS organisation.

This marks a major milestone in the delivery of neighbourhood health services in Bexley, as set out in the Government’s 10 year Health Plan and London’s Target Operating Model and Case for Change.

Integrators are not replacements for place-based care partnerships, which continue to play a vital role in delivering local ambitions. Instead, integrators provide the infrastructure needed to enable integrated neighbourhood teams to work effectively, ensuring services are joined up, proactive, and tailored to local community needs.

This is a positive step forward for Bexley. By appointing Oxleas as the host of our integrator, we are ensuring that neighbourhood services can work seamlessly together to support residents and ensure they receive the right care in the right place, at the right time.

This approach will help us to provide more proactive, personalised care – particularly for people living with frailty, multiple long-term conditions, and children with complex needs.

Over the coming months, partners across Bexley will be working together to develop integrated neighbourhood services, which will improve the health and wellbeing of residents across the borough.
