  • ensure you have the information and support you need to make an informed choice about your care and support options
  • provide the right information and support to enable you to find care and support options​
  • have knowledge about local care, support and wider community networks across the local area
  • give you the information you need to help you to understand the cost of care and support, financial assessments and options available to you

As commissioners we will:

  • develop flexible and creative care offers​
  • develop the care and support market around your preferences, choices and needs
  • stimulate the market to ensure there is choice
  • develop creative care and support options including community networks of support
  • listen to local people when developing the care and support market​
  • consider people's preferences and choices when developing the market within available resources

To monitor how we are doing we will:

  • use quality checkers to independently review the quality of our services
  • undertake audits of quality assessment and support planning, and use case studies in line with our vision and priorities​
  • continue to track our performance, while working to improve our personalisation measures - Adult Social Care Outcomes Framework (ASCOF), Self Directed Support (SDS) and Direct Payments (DP) which allows us to offer choice in how care and support is provided
  • monitor feedback from local people through our Friends and Family Test
