Active Travel

Travel that involves physical activity, such as walking or cycling, which improves health and reduces air pollution. 

Age-Friendly Communities

Communities designed to support healthy ageing by ensuring accessible housing, transport, services and opportunities for social participation. 

Artificial Intelligence (AI)

Technology that enables computers to analyse large amounts of data, recognise patterns and support decision-making. 

Bexley Care Finder

An online platform that helps residents locate health, care and community services available in Bexley. 

Bexley Wellbeing Partnership (BWP)

A partnership of organisations across health, care, voluntary and community sectors working together to improve health and wellbeing in Bexley. 

Carbon Capture, Utilisation and Storage (CCUS)

Technology used to capture carbon emissions from industrial processes and either store them or reuse them to reduce environmental impact. 

Child-Friendly Bexley

An approach that aims to make Bexley a borough where children and young people can grow up healthy, safe and supported. 

Connected Bexley

An online directory providing information about local services, activities and community support. 

Co-production

Working collaboratively with residents and service users to design, deliver and evaluate services. 

Digital Health

The use of digital technologies such as mobile apps, artificial intelligence and remote monitoring tools to improve health services and health outcomes. 

Digital Health Champions

Individuals who help residents build confidence and skills in using digital services and technologies. 

Digital Inclusion

Ensuring people have access to digital devices, internet connectivity and the skills needed to use digital services. 

Early Intervention

Providing support at an early stage to prevent problems from becoming more serious. 

Health Impact Assessment (HIA)

A structured process used to evaluate the potential health effects of policies, programmes or developments. 

Health in All Policies

An approach that ensures health and wellbeing are considered across all areas of public policy. 

Health Inequalities

Unfair and avoidable differences in health outcomes between different groups of people. 

Integrated Care

A coordinated approach where health and care organisations work together so people receive joined-up services. 

Integrated Care System (ICS)

A partnership of NHS organisations, local authorities and voluntary sector organisations working together to improve health outcomes and coordinate services. 

Integrated Neighbourhood Teams (INTs)

Multidisciplinary teams delivering coordinated health and care services for residents within neighbourhood areas. 

Joint Strategic Needs Assessment (JSNA)

An assessment of the current and future health and wellbeing needs of the local population used to guide service planning. 

Life-Course Approach 

A public health approach recognising that health outcomes are influenced by experiences across a person’s entire life. 

Making Every Contact Count (MECC) 

An approach where frontline staff use routine interactions with residents to encourage healthier behaviours. 

Micro-volunteering

Short and flexible volunteering opportunities that allow people to contribute in small, manageable ways. 

Population Health

The health outcomes of a group of people and how those outcomes are distributed across the population. 

Population Health Management (PHM)

A data-driven approach used to identify population health needs and target interventions to improve outcomes. 

Remote Monitoring

Technology that allows healthcare professionals to monitor patients’ health remotely using digital devices. 

Social Prescribing

Connecting people to non-medical support in the community, such as exercise groups, arts activities or advice services. 

Sexual and Reproductive Health (SRH)

Healthcare services focused on contraception, sexual health and reproductive wellbeing. 

Tech Hubs 

Community locations providing internet access, digital devices and support to improve digital skills. 

Very Brief Advice (VBA) 

A short evidence-based conversation used by health professionals to encourage smoking cessation. 

Wider Determinants of Health 

The social, economic and environmental conditions that influence health, such as housing, education, employment and income. 

Whole-System Approach 

A coordinated strategy involving multiple organisations working together to address complex health challenges.
